Notice the pattern: the most "lifestyle-friendly" specialties — Dermatology, Radiology, Ophthalmology — are also consistently the hardest to get into through NEET-PG. That's not a coincidence; it's simple supply and demand, and it's worth planning around rather than being surprised by.
Super-Specialization (DM / MCh)
After MD or MS, some doctors go one level deeper — into DM (Doctorate of Medicine, for medical branches) or MCh (Master of Chirurgiae, for surgical branches). This adds another 3 years, gated by its own competitive entrance exam, bringing total post-MBBS training to roughly 6 years for a super-specialist.

It's a real commitment: roughly a decade of formal training after NEET-UG before independent super-specialist practice begins.

Every one of these pathways is a multi-year commitment layered on top of MBBS, not a shortcut around it. The upside can be real — specialist physicians in the US, for instance, are commonly cited in the $300,000–$600,000+ a year range — but that ceiling sits at the end of a long, expensive, uncertain road, not at the start of it.

Salary Expectations

Government service tends to pay more in the early years — once housing, pension, and job security are counted in — while private practice and international careers open a much higher ceiling later, after specialization is complete. Neither path is objectively "better"; they trade off differently across a career.
